DCM | General Waiver + Liability Release

    In consideration of Dalraida church of Christ (hereinafter "Dalraida") located at 3740 Atlanta Highway, Montgomery, Alabama, furnishing facilities, services, and/or equipment to enable me to participate in activities promoted by the college ministry, I agree to the following:

    I understand that travel is sometimes necessary and part of these activities and I hereby acknowledge and willingly assume all risks and hazards that may arise if riding on a church bus provided by Dalraida or in private vehicles, should it be necessary for an activity.

    I assume full responsibility for my behavior during any activity and will follow all rules that have been established by Dalraida for an activity or event. I understand that if I were to violate any rules, I may be asked to leave the activity. I also acknowledge and agree that should I cause any damage to any property belonging to Dalraida I will assume responsibility for any repairs or replacement necessary.

    I voluntarily agree to release, discharge, and hold harmless Dalraida, its officers, agents, servants, and/or employees for any and all claims of liability to the fullest extent authorized by law for personal injury, property damage, or wrongful death arising out of negligence, fault, recklessness, or any other act or omission of Dalraida and/or and its officers, agents, servants, or employees that cause me illness, injury, disease, death and damages of any nature in any way connected with my participation in any activity.

    I also agree to release and discharge Dalraida from any act of omission or negligence in rendering or failing to render any type of rescue, emergency, or medical services. In signing this document, I fully recognize and understand that if I am hurt, die, or my property is damaged, I am giving up my right to make a claim or file a lawsuit against Dalraida, even if they negligently or by some act or omission cause personal injury or damage property.

    I further agree to hold harmless, defend, and indemnify Dalraida from all defense costs, including attorney’s fees incurred in connection with claims for bodily injury, wrongful death, or property damage that I may have caused to spectators or other third parties, whether negligent or not, in the course of my participation in any activity.

    I hereby attest that I have read this release, knowingly waive any other advisement on the potential dangers/risks when engaging in activities at Dalraida, assume all risks associated with activities at Dalraida, and am fully aware of and understand the terms and the legal consequences of signing this release. I knowingly and intentionally agree to this complete and unconditional release of all liability to the greatest extent allowed by law against Dalraida. I also agree that if any portion of this release is held invalid, it is agreed that the remainder shall, notwithstanding, continue in full legal force and effect.
