Please select which type of access device you would like to have issued. The card is the size of a credit card and can fit in your wallet and the fob is a small device that can be put on a keychain. If you have an access card or key fob for your work, you can probably use that for church access as well. Select that option below and we will try to do that instead of issuing you another card/fob.

I acknowledge that any issued access card or key fob belongs to Dalraida church of Christ and must be returned upon request.
I understand that individuals, including those members at Dalraida, will be issued cards based on the approved needs of the individual for access.
I agree that if I lose my issued card/key fob I will agree to pay for a replacement card ($10) and any issued access card/key fob will be deactivated upon termination of this request.
I agree that I will not loan my access card/key fob to another person.
